Below is a questionnaire which this office requests you fill out to the best of your ability returning it to our office via fax, mail or email to mrendon@wagnerjones.com. The facts contained in your completed form will assist this office in determining whether or not we can assist you in this matter.

Additionally, please remember to include witness statements (include the name, address and phone number of each witness) which need to contain remarks and/or actions taken against you in support of your claim.

Upon receipt of the completed questionnaire an attorney from our office will review the documents and someone will contact you when a decision has been made regarding your case. Please remember this process may take a couple of weeks.
